- Registration form: upon registration, you will be required to provide us with personal information such as name, telephone numbers, postal and email address, profession, GP contact details and insurance details when applicable. That information will only be processed by our designated staff under the respective lawful basis for processing it and will never be shared with 3rd parties without your consent.
- Medical information collected during treatment: This relates to any medical history, examination and treatment notes collected in session, as well as any medical information provided by your GP, Consultant or any other healthcare provider relating to your treatment. This information will only be processed by designated staff under the respective lawful basis for processing special category data for the purpose of providing healthcare and treatment.
How long we keep your data for:
Following the Chartered Society of Physiotherapy’s recommendations, we keep medical records for eight years from the date of last treatment for adult records, and for children eight years after their 18th birthday. Your contact and attendance details may be kept for longer in digital format unless requested otherwise by you.
We are committed to ensuring that your information is secure. In order to prevent unauthorised access or disclosure we have put in place suitable physical, electronic and managerial procedures to safeguard and secure the information we collect.
